The Admissions Coordinator accepts referrals for respite, residential and educational services 24 hours a day, seven days a week. For residential inquiries please call 217/585-5182. For more information about educational opportunities, please call 217/585-5122. Families, schools, social workers, PAS agents, mental health workers, case coordinators and clergy may make referrals. The Hope Institute serves children and youths with multiple disabilities between 5 and 21 years of age.

The Admissions Coordinator, along with a multi-disciplinary team that includes professionals representing education, residential, and health services, review referral packets and oversee the referral and admission process.

Residents of Illinois receive funding through one or more of the following: Illinois Department of Human Services, Illinois Department of Children and Family Services, local school districts and private pay. Private pay is also accepted for all out of state admissions.

In order to make a thorough assessment of a child the referral packet must include:

  • a cover letter indicating reason for referral and presenting problem
  • medical history
  • current physical and list of current medications
  • most recent Individualized Education Plan (IEP) and 3 year case study
  • social history
  • psychological evaluation including diagnosis and full scale IQ or functioning level
  • information regarding behaviors and the Behavioral Intervention Plan (if appropriate)

Services are offered to all children without regard to race, religion, national origin, age, sex or disability.
